手数料

利用目的の提示、保有個人データの開示又は第三者提供記録の開示のご請求につきましては、１回の請求につき１,５００円の手数料をいただきます。保有個人データの追加・訂正・削除及び保有個人データの利用・消去・第三者提供の停止につきましては、無料で承ります。保有個人データが存在しない場合又は法令によりご請求に応じられない場合であってもお支払いいただきました手数料は返金できませんのであらかじめご了承ください。お支払い方法については、お問い合わせいただきましたときにご案内させていただきます。

回答方法

ユーザー本人の本人確認、代理人の本人確認（該当する場合）、その他関連書類の確認（該当する場合）及び入金確認（該当する場合）が完了した後、当事務所は電子メール又は郵送によりご請求内容に従って回答いたします。
